The maxillary sinus area has always been taken into consideration with a certain "reverence" by the vast majority of dentists, due to the apparent difficulty of accessing this area and the delicacy of the membrane of Schneider which covers the entire internal surface. The bone quality present in this area makes the insertion of implants, with both conventional and post-extraction techniques, even more complex and with reduced survival rates. On the other hand, following the guidelines of the CMI concepts, it is possible to partially anchor the implant in the cortex of the maxillary floor, thus obtaining considerable primary stability.

The lecture will uncover the benefits that can arise from utilizing Neobiotech IS-II and IS-III implants combined with CMI concepts, in this particular area of the oral cavity. The presentation will also detail prosthetic loading protocols in relation to the quantity and quality of residual bone in order to obtain high survival rates.

Every day in our consultations we handle patients whose main reason for consultation is oriented to immediate treatment that they require. A situation that we solve with the implementation of optimal macrodesigns of implants (CMI) and its connections. In the past decade, NEOBIOTECH implants have shown the “AnyTime Loading Concept” to the implant dentistry world. They optimized implant design, surface treatment, and drilling protocols for this concept.

They also claim that they can eliminate the dip between the primary & secondary stability of dental implant healing, especially during the second week to the eighth week after dental implant(s) placement.

I will take the audience to follow my lecture, which shows the case examples in various bone densities of dental implant cases, including different drilling protocols in each. I record the dental implant stability in 2-week intervals, till the eighth week after dental implant placement. ANYCHECK is the tool for my initial stability test in IST values.

The primary stability of an implant, resulting from the mechanical interconnection between the implant and the bone, is one of the most critical factors in long-term success. The absence of micro-movements beyond the tolerable limit is essential for successful osseointegration.

After insertion of an implant, bone remodeling causes a decrease in primary stability. As long as there is not enough secondary (or biological) stability, this stability gap makes it inadvisable to load the implant. High primary stability at the time of surgery does not necessarily mean that it will remain high over time, as it is affected by the bone remodeling process around the implant. There are several factors that can increase bone necrosis and the extent of this remodeling, such as increased temperature, excessive torque, among others. In this presentation we will address several biological principles that must be taken into account during implant bed preparation, in order to reduce aggression to the bone tissue and maintain primary stability over time, allowing the implant to be loaded at any time. We will analyze the most recent scientific literature, summarizing the critical factors so that the clinician can optimize their protocols.

Achieving aesthetics with implants is a challenging and demanding procedure. To create implant restorations with harmonious gingival contour that emulate nature is a fusion of science and art. Understanding the biologic and physiologic limitations of the soft and hard tissues, together with the provisional restoration ideal shape, will facilitate predictability in simple to complex esthetic situations.

The aesthetic success of a prosthesis on implants is judged almost exclusively by its imperceptible integration with the adjacent elements and by its appearance as natural as possible. To achieve these objectives, a correct treatment planning must be carried out, taking into consideration all the decisive parameters to obtain an ideal result.

During the lecture the key determinant factors for an optimal and lasting aesthetic solution of immediate load cases will be highlighted, from the study of the case to the surgical phase, with particular attention to the management of the prosthetic emergence profile through the conditioning of the soft tissues, and its ideal occlusal plan, up to the realization of the provisional and the definitive restoration. Particularly attention will be then caried out to the occlusal plan design to preserve the bone from its reabsorption, and both the implants, the prostheses restorations and the prosthetic components from their fracture or other failures according to what are the most current orientations of the scientific literature.

What are the normal loading protocols we have been taught from the father of implant dentistry? Osteotomy preparation at high rpm, placement of implant, bury implant, 3-6 months later uncover implant and place healing abutment, 1-2 months later begin restorative impressions. We have been taught that since 1950, why do we still adhere to these protocols? What has changed in dentistry since 1950? Mercury is not beneficial for our health. Sedation can be used to help patients with anxiety. Local anesthetics are safe to use on every patient. Periodontal disease does not mean you must extract every tooth in the mouth. Everything has changed except implant loading times. When do our patients want implants to be loaded? At the time of placement, they are expecting to have a new tooth, not just the root of a tooth. Why can’t we give our patients what they want? Because we are working in the 1950’s with our dentistry.

This presentation challenges the status quo and advocates for a shift to meet patient expectations.
